Commissioner of Oaths

A Commissioner of Oaths is a person authorized by the Province of Ontario to administer oaths, solemn affirmations, and declarations for legal or official purposes. Commissioners do not verify the truth of the statements in a document — they only confirm that an oath or affirmation was properly administered.

In the Township of Manitouwadge, the Clerk, Deputy Clerk, Treasurer, and Deputy Treasurer are authorized to act as Commissioners of Oaths within the municipality.

Commissioner of Oaths services are available Monday to Friday, 9:00 a.m. – 4:00 p.m.

  • Treasurer’s Office: Walk‑ins are welcome during regular business hours. Availability may vary depending on workload.

  • Clerk’s Office: Appointments are required to ensure the Clerk is available to commission your document.

To have a document commissioned, you must:

• Appear in person
• Bring valid government‑issued photo ID (driver’s licence, passport, etc.)
• Bring the completed document, unsigned
• Sign the document in front of the Commissioner

Commissioners cannot provide legal advice, complete forms for you, or supply blank affidavit forms. If you require an interpreter, you must bring your own.

The Commissioner will sign documents relating to the following, but not limited to:

  • Residency Documentation
  • Insurance Claims
  • Certify Documents as “True Copies” (See Certified True Copies Section)
  • Certification of Identifiable Individual’s Signature
  • Real Estate Documents supplied by lawyer
  • Any government-related forms, including but not limited to:
    • Legal Name Change Application
    • Name Change and / or Sex Change documents and application
    • Declaration Affirming Parentage
    • Election to Change Name of Child under the age of 12 (Service Ontario form)
    • Birth and Death Information Applications as prescribed by Statute
    • Delayed birth registrations
    • Application to amend birth registrations
    • Lost passport
    • Statement of Conscience or Religious Belief – Immunization of School Pupils Act
    • Vehicle Ownership Transfers – Sworn statement for a transfer of a used vehicle in the Province of Ontario vii. Pension Purposes – Attestations a person is alive and confirming residency viii. Common law or single status affidavits ix. Construction Act documents for progress and/or completion x. Municipally-Related Documents (In-house) and Municipal planning applications related to matters within the geographical limits of the Municipality

The Commissioner CANNOT and WILL NOT sign documents relating to the following:

  • Wills, Living Wills, Codicils (amendments) to Wills 
  • Powers of Attorney 
  • Certified True Copies (See Certified True Copies Section) 
  • Divorce, Separation or Cohabitation documents 
  • Custody documents 
  • Transfer of land documents 
  • Real Estate documents not supplied by a lawyer 
  • Estate documents 
  • Documents related to finance (Banking information, RRSP etc.) 
  • Court, Legal and/or Civil issues related documents 
  • Consent letters for children travelling out of country

Some documents require a Notary Public, not a Commissioner of Oaths. If your document specifically states “Notary Public,” you must contact:

  • A law firm licensed to practice in Ontario
  • A Justice of the Peace
  • Service Ontario
  • Another authorized notary service

Commissioners cannot notarize documents.

The Clerk’s Office may certify true copies of municipal records only.

Commissioners cannot certify true copies of other documents such as passports, diplomas, or identification. If you still request certification, you will be advised that the receiving organization may not accept it.

The original document must be provided.

The Township does not provide interpreters. If you require interpretation, you must bring your own interpreter to the appointment.

Commissioners may refuse to sign a document if:

  • They cannot confirm your identity
  • They are unsure about the content of the document
  • The document appears incomplete or inappropriate

If we are unable to sign your document, you may be referred to:

  • Service Ontario
  • Court Office
  • Justice of the Peace
  • Local law firms or notary services
